If you are a new client with us, please print and complete the appropriate form for each service you are receiving. Please give it to your therapist to review on your first visit. These health forms ensure that your therapist will give you an appropriate course of treatment for your individual health history. You may email the forms in advance to us at:

Skin Health Assessment - Complete when receiving facial services.

Hair Removal Assessment Form - Complete when receiving waxing services.

Pregnancy Intake Form - Complete if you are pregnant for your Pregnancy Massage session.

Ashiatsu Release Form - Complete when receiving Ashiatsu DeepFeet Bar Therapy.

Cupping Release Form - Complete when receiving Cupping or MediCupping.

Reflexology Form - Complete when receiving Reflexology.
